One document per trimester from July 2007 up to the time you send the application 8. Proof that you were in the USA before your 16th Birthday 9. One document every year from the time you came to the USA to 2006 And don't write any fake social security numbers where it's asking for any used. They are asking for any ssn given by the social security administration only. Try to google a DACA cover letter to customize for you and put it on the front of all pages. Make copy of all docs/evidence you will send for your records
