"You must sign your application in the signature block if you are 14 years of age or older. Your parent or guardian may sign their name in the signature block if you are younger than 14. I have called twice asking what they mean by this. The first time the lady who answered told me to write a letter saying that I was preparing the papers for myself and that that was my signature. I did that, returned it, and then it was returned to me again. I called back, and the guy who answered didn't understand why and said he didn't know how to help me. For my application I used my First Name, Middle Name, Last Name-Other Name. Do they want meto use all fo them then? I asked the man and he know what to tell me. Can anyone help me at all? Give me any tips or advice? I have attached a picture to help visually.

My form was returned for the following reason: "You must sign your application in the signature block if you are 14 years of age or older. Please sign and send the application...." I didn't write anything there at all I placed my signature, phone number and date after the paragraph that begins with "Your certification..." and ends with "Question 16" I sign my name with my first name and last name, not my middle name or my hyphenated last name. My mother has my same name, it's in my BC, are they some how thinking that's hers? Oh well, I'm going to call but just wanted to throw this in here. Did anyone else have a problem like this? Edit: Could it be that the C and Z in my signature crossed the line a little to the "signature of person preparing form...?"

I remember when I was wating for an envelop full of coupons... I coupon, and when I checked the tracking number I remember it saying "sorting" and having a zipcode different from mine. I was freaking out because it had been more than a week already and I had no received it yet. My deals were going to expire, I was angry. > Turns out it had arrived a few days ago but my sister put all the mail away, without telling anyone, when she had company over. Don't freak out, I'm more than sure they already received it.

I sent it on Feb. 14, and checked the mail monday night and there it was! I just don't understand what they're asking bc I didn't sign anything under "signature of person preparing form, if other than above:" since I was the one filling out my own papers. I just called and it's asking for my receipt number, mine has three letters at the beginning I can't type them in so it's telling me that I don't have enough numbers and I don't know what to do.
