  11. You dont need to be currently enrolled in school if youre older than 18, theyre referring to people still going to high school.
  12. A good document to prove that you were in the US on june 15 2012 is also a utility bill and yes medicaid should be an acceptable document, just make a note at the bottom explaining why youre sending it in...
