Ok so I'm still filling out parts of the application when i had a thought if im filling it out wrong...Since i have two last names on my birth certificate but only use one, when i fill out my application im filling it out like i normally do with one last name but on this do i have to put both last names since that is how it shows up on my birth certificate?

Hi, My name is Sam, I was only 6 months old when I was brought here. After a while I visited Mexico again when I was about 8 or 9 years old, returned a few months later and I've been here since I returned back to the US at the age of 10 or so. I am now 19 going on 20 in two months. I've been waiting for an opportunity like this and now I'm so excited to do things and get things done. Although I am nervous and freaking out a bit, as long as I get the chance to change things, I am willing. Wish me luck! XD -Sam

Does the passport have to be valid? I believe mine expired some time ago, so what do i do? Do i need something else for identification?
