You don't have to submit evidence exactly dated on June 15th, 2012. The evidence can be dated shortly before or after, maybe like within a week or two. Find the closest dated evidence you have and send that in. G-1145 is a form that you fill out so that you are notified by e-mail or text message whenever the lockbox gets your documents. They send you two messages and in those messages you will find two Receipt Numbers, one for the I-821D application and the other for the I-765 application. These Receipt Numbers are used to check the status of your case on the USCIS website. It isn't required to fill out that form and it doesn't matter if you do or not, you will still be sent two Receipt letters in the mail that say the same thing that the e-mail/text says. -

Yeah, but these are the same people that were letting DACA applicants make Service Requests on their I-765 application after just 3 months of waiting without having their I-821D approved even though the USCIS website stated that it couldn't be done. I'm sure if you call USCIS and ask if you could walk-in for your Biometrics appointment they will tell you no, but of course many people have done it without a problem. USCIS reps say many different things so I see why some people would be suspicious, but they're not always correct. -

He wasn't asked for personal information nor money over the phone though. He was asked for additional proof and was told that the facebook evidence he sent as a response to his RFE wasn't valid. That's not something a scammer would know. -
You should send a copy of the original birth certificate and the corrected passport.

Hmmm... I would send both of them and a letter explaining that the birth certificate you sent in had a mistake on it, that your real name is Osas Odion Juan, as shown on the original birth certificate. Does your other evidence also have the wrong name? Like school documents? Or is it just your birth certificate.

Left Usa 2007, Obtained Visa B-2, Residing In The Us With Permit Is Dream Act Still Possible?

There currently isn't a Dream Act. If you mean Deferred Action for Childhood Arrivals (DACA), then I don't believe you're eligible. To apply for DACA, you must meet 7 guidelines, one of them being that you must have continuously resided within the U.S. since June 15, 2007 up until the time of filing your application. You say you left in 2007 and came back in 2011, that is a 4 year gap in the continuous residence guideline, so you would be denied. Another requirement is to have been here in the U.S on June 15, 2007, so I don't know if you were or not. Is your VISA expired and have you renewed the I-94 since you obtained the first one? -
There's 4 different tests. The one I took didn't have any questions from the website practice tests. If you guys have a smartphone, you can download the app called DMV NOW which has like 50 or 60 practice questions for the written test.

As far as I'm concerned, you don't need the card, just the physicians recommendation or prescription. The card is voluntary and it's just to show it to police officers in case they stop you and you have marijuana on you because some don't accept just the physician's recommendation as proof.

To change your address you must fill out form AR-11 and mail it to USCIS. If you already called and did the Service Request, just wait for the response to that. You should be getting it probably within a week.

Keep in mind that you don't have to send evidence to cover the whole year of 2007, just from June 15, 2007 forward. If you literally have nothing for the remaining 6 months of 2007, you will most likely get an RFE. You might be able to get away with sending just a few documents for those 6 months, so look hard to see if you can find anything.

You should usually get it within 2-3 weeks. You should give USCIS a call at their 1-800 number and listen to the phone options and dial the ones about RFE issues so that you can speak to a representative about this. -

That's usually what happens. The I-765 updates a few hours before the I-821D application does. -
Yes. What I meant is that we're going to have to re-apply for our DACA renewal. The requirements nor the application for that renewal aren't out yet, so no one knows what the process is going to be, so if for some reason someone isn't able to renew their DACA, they won't be able to renew their work permit either.

Only if your DACA is renewed. You don't qualify for a work permit without DACA.
