Question about I-765

on the I-765 where it says date last entry i wrote 10/31/2004 which is of course the last time i entered the states.

I do have my last permit (I-94) on question #10 asks about the I-94 #

My dilema:

on the I-821D question 13 asks about the initial entry date (2000). On question16a. asks about the I-94 i decided to leave blank because the I-94 i have is like i said from the last time i entered not the initial entry. But on the I-765 question #10 ask about the I-94 # again, should i leave blank or should i go ahead and write the # down???

Help dont want to make an error and.get rejected

so did you did your I-94 after you entered the US??

i wouldnt leave it blank because your initial entry date is different then the last time you entered the US. and if the last time you entered the US, you had an I-94 then you were allowed to enter legally.

also when did you're I-94 expire?

Im assuming you are in dilemma about that because you don't have the I-94 fi yor initial entry, right? Do you have some evident to show you were here before 2004, possibly from 2000? If yes, write down your initial entry date 2000. Even if you don't have anything to show your presence from 2000, I would put down the initial entry. If you leave the space blank, your application will come back to you in the form of RFE.
