On the Social Security Card application there is a section where it says "NAME TO BE SHOWN ON CARD" and under that it says "FULL NAME AT BIRTH" http://www.socialsec...online/ss-5.pdf What I'm wondering is if anyone here applied for a Social Security Card and didn't include both last names or left out a middle name. I ask this because previously I thought this wasn't possible since I read several people who were told to write down their full name as it appears on their EAD, but according to the SSA guidelines (POMS) we can omit our middle name and suffix and since our second last name isn't used in the U.S. it would count as a suffix. https://secure.ssa.g.../lnx/0110205120 So, did anyone get their Social Security Card with a different name than shown on their EAD?
