Cheeselover 0 Posted February 27, 2013 My form was returned for the following reason: "You must sign your application in the signature block if you are 14 years of age or older. Your parent or guardian may sign their name in the signature block if you are younger than 14. Your signature or parent's or guardian's signature in the "Signature of person preparing form, if other than above" block is not a valid signature for this purpose. Please sign and send the application...." I didn't write anything there at all I placed my signature, phone number and date after the paragraph that begins with "Your certification..." and ends with "Question 16" I sign my name with my first name and last name, not my middle name or my hyphenated last name. My mother has my same name, it's in my BC, are they some how thinking that's hers? Oh well, I'm going to call but just wanted to throw this in here. Did anyone else have a problem like this? Edit: Could it be that the C and Z in my signature crossed the line a little to the "signature of person preparing form...?" Share this post Link to post Share on other sites

Cheeselover 0 Posted February 27, 2013 I sent it on Feb. 14, and checked the mail monday night and there it was! I just don't understand what they're asking bc I didn't sign anything under "signature of person preparing form, if other than above:" since I was the one filling out my own papers. I just called and it's asking for my receipt number, mine has three letters at the beginning I can't type them in so it's telling me that I don't have enough numbers and I don't know what to do.
