pswa83 302 Posted June 19, 2013 you get the form when you actually go to the appt. not before. it's super simple. it just ask the basic info. name, place of birth. it also ask for your receipt numbers. so if only one of them are at the bottom of your bios letters, write the other one day either on your cell phone. remember to take your photo ID with you when you go and the bios letters. the process is usually pretty fast and easy. make sure you look good cuz the pic they take will be the one on your EAD. lol Share this post Link to post Share on other sites
