zobo411 1 Posted August 28, 2013 RFE !!! "Uscis records and/or evidence you submitted indicate that your name is Odion Osas Juan . You indicated on your request for Daca your name is Osas Odion juan. Submit documentary evidence to establish your true name . Such evidence may Include a birth certificate , marriage certificate , passport etc etc." Odion is my middle name And Osas is my first name Juan is my last name on my birth certificate it states odion as my first name and osas as my middle name.. which is a mistake.. The birth certificate was sent to uscis with my daca application.. Apparently my mom has another birth certificate that states my correct name osas odion juan.. this birth certificate was made long time ago when my mother thought she misplaced the first one. My question is that should i send out the birth certificate with my correct first name OSAS ? or send in both ? with an explanation . HELP !! Share this post Link to post Share on other sites

I would send both of them and a letter explaining that the birth certificate you sent in had a mistake on it, that your real name is Osas Odion Juan, as shown on the original birth certificate. The evidence that i sent in that had my middle name as my first name was My expired B2 visa and Expired Passport & Birth certificate .. My New Passport has my correct first name and my Last name OSAS Odion Juan so as my school records and other recent documents.. Share this post Link to post Share on other sites
