Ly_vs 0 Posted October 5, 2012 On I-821D page 2 Other Names Used question 12b.First name ..........my name ends with a "y" and everyone spelled it with an 'I" at the end its spelled like that in school records n every proof i have except my Birth Certificate n my ID so how do i explain that in page 6 in additional info ? JoseG 383 Posted October 5, 2012 Put your original name as it is on your birth certificate on Part 1.
